An exciting opportunity has arisen within our Community Response Team, for an enthusiastic, motivated and experienced Senior Physiotherapist to join our team based in York.

This is an exciting opportunity to join a dynamic and developing patient focused rehabilitation service, supporting people in their own homes.

You will be responsible for providing both holistic assessments as well as specialist physiotherapy treatment and management of patients, providing rehabilitation to patients in their own homes as part of a Multi-Disciplinary Team.

The team works closely with all partners, including hospital wards, GP’s, A&E, Social Services and the voluntary sector, supporting hospital discharges and preventing unnecessary admissions.

The post holder will be required to work closely with the Out of Hospitals Directorate team. You will have the opportunity to undertake a wide variety of tasks. The successful applicant must have excellent communication and organisational skills, work flexibly and be a strong team player. You will be expected to provide support to students and junior members of staff.

The ability to transport yourself and equipment across the locality during the course of a normal working day is required for this vacancy.
