York Teaching Hospital Foundation Trust is looking to recruit an enthusiastic and self-motivated Discharge Liaison Officer. The integrated complex discharge hub provides a service to patients and customers across York Hospital, White Cross Court Rehabilitation Hospital, St Helens Rehabilitation Hospital, Scarborough Hospital, Bridlington, Selby New War Memorial Hospital, St Monica's Easingwold.    

About the role:

You will join a developing inter-agency team working closely with; ward staff, Social Care, Continuing Health Care and the voluntary sector. Therefore you must be able to demonstrate collaborative working as well as being able to build effective relationships with colleagues from other healthcare professions.

The team is developing in line with national guidelines to facilitate complex discharge planning with an ethos of ‘home first’ and the successful candidate will be employed as an integrated team member with our partners.

This role entails working closely with a wide multidisciplinary team to facilitate the timely discharges of patients from the hospital environment.

 The work base for this post is York but the successful applicant will be expected to provide cross site cover.

You will benefit from working within a small, friendly, experienced, pro-active team who are looking for a colleague with a positive approach, and has a flexible and patient centred approach.

Please note the hours available for this post are to be worked over a 7 day period

What are we looking for:

The ability to escalate concerns via appropriate channels of escalation is also essential in this role.

The successful candidate will be required to use a variety of computer applications and should be confident in IT skills.

Please note: The ability to transport yourself and equipment across the local area during the course of a normal working day is essential for this role.
