…to help make us a valued and trusted partner within our care system, that delivers safe effective care to our local population.
We would welcome applications from highly motivated individuals to join our dynamic and enthusiastic team of geriatricians, working within our wider multi-disciplinary team at York Teaching Hospitals NHS Foundation Trust.
Within the Directorate of Older People’s Health Services we have developed and taken steps towards achieving our vision of; ‘Becoming a centre of excellence in the care of older people’. In order to deliver this goal we have embarked on an ambitious programme of quality improvement and service modernisation which is helping us to transform the care we deliver to frail older people within our localities.
The directorate of Older People’s Health encompasses a number of specialist services, including a frailty assessment unit within our ED, dedicated frailty acute care and Short stay facilities, Specialist hip fracture and orthopaedic liaison, Hyper-acute care and stroke rehabilitation services and the recent redevelopment of a dedicated dementia and delirium unit. We also provide clinical input into two of our local community hospitals alongside working closely with local GP’s providing outpatient services and in-reach to local care homes.
As a directorate we have a track record of utilising innovative models of care to help us to improve quality and clinical outcomes for the benefit of our patients and local communities.
To support us in delivering our vision we are now seeking to further enhance services in Acute Frailty, Surgical Liaison and to strengthen the role of community geriatrics in our wider integrated care agenda.
Applicants should be fully registered on the UK GMC Specialist register or up to 12 months within their expected date of receipt of CCT / CESR or EU equivalent certification at the time of interview. Applications are welcome from those unable to work full time or those wishing to job share. Relocation packages are available for consultants looking to move to Yorkshire to take up roles within our organisation. We support a flexible approach to working patterns and delivery of job planned activities (such as 10PA's worked over 4 days) and would discuss this requirement with any interested or successful candidate.
Further information about the directorate and the roles and services we are looking to develop is included in the job description, person specification and attached directorate information pack.
