York Teaching Hospital Foundation Trust is looking to recruit two enthusiastic and self-motivated Discharge Liaison Specialist Nurses/ Occupational Therapists/Physiotherapists. The integrated complex discharge hub provides a service to patients and customers across York Hospital, White Cross Court Rehabilitation Hospital, St Helens Rehabilitation Hospital, Scarborough Hospital, Bridlington, Selby New War Memorial Hospital and St Monica's Easingwold.    

Please note that for this post we are welcoming applications from both registered Occupational Therapists/Physiotherapists and Nursing professionals.

About the role:

You will join a developing inter-agency team working closely with; ward staff, Social Care, Continuing Health Care and the voluntary sector. Therefore you must be able to demonstrate collaborative working, as well as being able to build effective relationships with colleagues from other healthcare professions.

The team is developing in line with national guidelines, to facilitate complex discharge planning with an ethos of ‘home first’, and the successful candidates will be employed as integrated team members with our partners.

As a Discharge Liaison Nurse/ Occupational Therapist/Physiotherapist, you will be expected to demonstrate a wide range of knowledge, skills and experience in relation the following:

  • Delayed Transfers of Care (DToC)
  • Continuing Health Care (CHC)
  • Choice of Accommodation
  • Patients who require long term care
  • Patient assessments in relation to requirements for rehabilitation and packages of care.

You will be required to have high level of sensitive discussions with patients and their families, and on occasion be able to diplomatically challenge conversations in order to achieve the best outcome for patients for a safe discharge.  

You will benefit from working within a small, friendly, experienced, pro-active team who are looking for a colleague with a positive approach, and has a flexible and patient centred approach.

Please note the hours available for this post are to be worked over a 7 day period

What are we looking for:

You should be a dynamic, forward thinking practitioner with the desire to develop and improve services. You must have relevant clinical experience, excellent communication and organisational skills.

The ability to escalate concerns via appropriate channels of escalation is also essential in this role.

The successful candidates will be required to use a variety of computer applications, and should be confident in IT skills.

Please note: The ability to transport yourself and equipment across the local area during the course of a normal working day, is essential for this role.
