We are pleased to be able to offer this exciting opportunity to join the Critical Care Outreach Team based at York Hospital. The Outreach Team is friendly and supportive with close links to our Intensive Care Unit and the associated Multi Disciplinary Teams. Our team deliver a high standard of individualised care to deteriorating and acutely ill patients throughout the hospital 24 hours a day. They ensure a timely and effective response to patients and support both medical and nursing colleagues.
We are looking for a highly motivated individual who is passionate about the care of deteriorating patients and who will enjoy the challenge of working in this demanding but highly rewarding role.
The successful applicant must be able to demonstrate excellent clinical skills and sound evidence based decision making. They will carry out advanced assessment and care of deteriorating patients leading to early intervention and treatment. They must have excellent communication skills and be able to work autonomously. Formal and informal education is an integral part of this role and the successful applicant will take an active part in education around the management of deteriorating patients. Induction and ongoing training will be provided to ensure competency and safety. The successful applicant will be involved in service development, improvement and audit.
