York Teaching Hospital Foundation Trust is looking to recruit an enthusiastic and self-motivated Band 7 Heart Failure Specialist Nurse working within the Vale of York.
About the role:
Based at our Clemanthorpe Health Centre, you will work as part of our Community based Heart Failure Team, liaising closely with cardiology and medical teams within our Acute and Community Hospitals, with options for both in-patient and daily IV diuretic therapy.
You will be required to manage a varied community caseload of patients with heart failure, using evidence based management strategies to seek the best outcomes to improve morbidity, mortality and quality of life and to provide palliative support to the deteriorating patient.
The role incorporates clinical assessment, titration of medications, education and support to patients and carers. Your role will also involve seeing patients at home and in clinic depending on need.
Please note: The ability to transport yourself and equipment across the local area during the course of a normal working day is essential for this role.
